Short-Term Independent Study

This is information for a short-term independent study request of 1 to 15 consecutive school days.

This independent study contract is different from the full-time PUSD Independent Study Program.

Process Overview 

  1. Student is to download and complete the cover sheet, including student & parent signature, and return it to your school counselor.
  2. School counselor will review and preliminarily approve the request for short-term independent study.
  3. Student must provide each of their teachers with a copy of the Student Work Assignment Record (SWAR) form (page 2) with all fields at the top filled in (except Date Work is Due to Teacher which is to be filled by teacher). Make copies of the SWAR form, as needed.
  4. Each teacher will complete the Assignment, Materials & Resources, and Learning Objectives portion of the form and provide the date all completed work is due. Teacher will sign, date and return the form to the student.
  5. Both the student and parent/guardian will review the forms and sign.
  6. Student keeps the SWAR forms as a record of their assignments due.
  7. At the end of the short-term Independent Study Contract period, student must submit their completed work to each teacher.
  8. If work was completed, teacher should check the All Work Completed box, sign and date.
  9. Student must return all signed SWAR forms to their school counselor by no later than 10 days after the student's return to school.
